Go to your Myetherwallet
click on "Add Custom Token" button.
Fill in all the fields:
Put 0xd4c435f5b09f855c3317c8524cb1f586e42795fa
in the "Address", "CND" in the "Token Symbol" and "18"
in the decimals field. Then click on "Save" button.

Actually if you will add wrong address - you will not see your CND balance.
