It appears to be a disk failure of some sort, our host said to us:
"We have had a storage fault in our NetApp cluster. This fault resulted in some file system errors in a few cases."
We have lost some non-critical data and we're working on restoring our server at this time. All user balances and account information is in tact due to our strong backup policies.

Working fine for me again, thanks for the quick fix and response!
