mine runs on 12.7 volt so 12,7/15=0.846 volts per chip

As I can see you also doesnt have best share on S5.

Any idea why ??

Yes, reduced cpu load to 20%, fresh cgminer 4.9.0 and nice  graphs 

I've made it. Based on latest official with fresh cgminer and some tweaks, include some additional software to render nice graphs   also includes right extranonce patch, flashed like official firmware via web interface.
But this is not an official firmware so for many people this is not an option
Btw, brick is impossible, if you had power outage when flashing, just load s4 firmware onto sd card and boot miner, after that flash s5 firmware again

No special knowlege or steps needed, just download firmware form nicehash(https://www.nicehash.com/index.jsp?p=software#asic), load it via web interface System->Upgrade, wait 90 seconds, done.
Also you can navigate to http://miner_ip/cgi-bin/graph.rrd to see some perf graphs. They collected locally in background every 60 seconds, stored inconfig folder so the stats will survive reboots.

Let me know if people (including time travellers) want mirrors on these, I'll download and keep copies.
